At Limewood Care Home, we work closely with healthcare professionals, local authorities and hospitals to support individuals with complex care needs. Our experienced team ensures every referral is handled with care, professionalism and attention to detail.

Our skilled multidisciplinary team, including registered nurses, clinical care practitioners and therapeutic support staff, collaborates with social workers, hospital discharge teams, case managers, NHS Continuing Healthcare assessors and local authority professionals to deliver consistent, person‑centred care from the very start.

The Referral Process

Referrers can contact our team to discuss an individual’s needs, allowing us to gather key background information and determine suitability. A full clinical assessment is then arranged, usually within the individual’s current setting, to understand their physical, cognitive and emotional needs.

Following assessment, we work collaboratively with all involved professionals to plan care and confirm placement. Once agreed, our team supports a smooth transition into the home, ensuring continuity of care from day one.

Specialist Dementia

Our dementia care service in Stafford supports individuals living with Alzheimer’s disease and other forms of dementia, including those experiencing behavioural and psychological symptoms.
